Miniscrew-Assisted Rapid Palatal Expansion
What is MARPE?
In short, MARPE widens an upper jaw that’s too narrow, creating more room for teeth and helping improve how the upper and lower teeth fit together. Its full name is Miniscrew-Assisted Rapid Palatal Expansion, but the idea is simpler than the name suggests.
The treatment uses a specially made device that fits against the roof of your mouth. Small, temporary screws hold it in place, allowing controlled pressure to be applied directly to the upper jawbone rather than relying mainly on the teeth.
This approach can be particularly useful for older teenagers and adults, whose jawbones have developed beyond the stage when traditional expanders are usually most effective.
MARPE isn’t suitable for everyone. Your orthodontist will examine your teeth and jaw, review your dental history and determine whether it’s an appropriate way to achieve the changes you need.
When might MARPE be considered?
When the upper jaw is too narrow, your teeth may struggle to fit together as they should. This can affect your bite, leave teeth crowded or cause the upper and lower teeth to meet unevenly.
MARPE may be considered for several concerns:
Narrow upper jaw
The upper jaw doesn’t have enough width to fit comfortably with the lower jaw.
Crossbite
Some upper teeth sit inside the lower teeth when you bite down, rather than slightly outside them.
Crowded teeth
A narrow upper jaw may contribute to teeth overlapping or competing for space.
Uneven bite
Differences in jaw width can affect how your upper and lower teeth meet when you chew or close your mouth.
The important distinction is that not every crowded tooth or uneven bite is caused by a narrow upper jaw. Your orthodontist will assess what’s behind the problem and whether widening the jaw with MARPE is the right approach.
How does MARPE work?
MARPE uses an expansion appliance that is supported by miniscrews placed in the palate. The appliance is activated according to a prescribed schedule to apply gradual expansion forces.
Treatment generally involves several stages:
1. Initial assessment
Your dentist or orthodontic provider assesses your teeth, bite, jaw relationship and oral health. Dental records and imaging may be recommended where clinically appropriate to evaluate your anatomy and help determine whether expansion is suitable.
2. Treatment planning
If MARPE is considered appropriate, your provider explains the proposed approach, expected treatment stages, potential risks, alternatives and estimated costs before treatment begins.
3. Appliance placement and activation
The expansion appliance is fitted and activated according to individual instructions. You may notice pressure or temporary discomfort as the appliance applies expansion forces. Follow your provider’s instructions and contact the practice if you experience significant pain or other concerns.
4. Stabilisation and further orthodontic treatment
Once the planned expansion has been achieved, the appliance may need to remain in place for a period to allow the result to stabilise. Further orthodontic treatment may be required to align the teeth and refine the bite.
Treatment duration and outcomes vary between individuals.
What are the potential benefits of MARPE?
For suitable patients, MARPE may help widen a narrow upper jaw, creating more space and improving how the upper and lower teeth fit together. Depending on individual treatment needs, potential benefits may include:
- Increasing the width of the upper jaw.
- Improving the relationship between the upper and lower dental arches.
- Helping address certain crossbite concerns.
- Creating a more suitable foundation for subsequent orthodontic treatment.
Results vary from person to person, and the amount of expansion achieved depends on individual circumstances. In some cases, a different treatment approach may be needed to achieve the desired outcome.
Is MARPE suitable for adults?
MARPE may be considered for selected adolescents and adults, including some whose jaw growth is largely complete. However, suitability depends on factors such as skeletal maturity, palatal anatomy, the severity of the jaw-width discrepancy and overall oral health.
Age alone does not determine whether MARPE is appropriate. A clinical examination, supported by suitable diagnostic records, helps inform the decision.
In some cases, alternative orthodontic approaches or surgically assisted expansion may be considered. Your provider can explain the options relevant to your circumstances.
What should I expect during MARPE treatment?
It is common to notice pressure, tightness or temporary discomfort during appliance activation. The appliance may also affect speech or eating while you adjust to it.
Good oral hygiene is important throughout treatment. Food and plaque can collect around the appliance, so careful cleaning and any recommended additional hygiene measures are essential.
Your provider will explain how to clean around the appliance, how to activate it if required, and when to attend review appointments. Contact the practice if the appliance becomes loose or damaged, or if you develop significant or persistent pain, swelling or other unexpected symptoms.
Frequently asked questions
Is MARPE painful?
You may experience pressure or discomfort during appliance activation and as your mouth adjusts. The level of discomfort varies between patients. Your provider can discuss what to expect and how to manage symptoms during treatment.
How long does MARPE treatment take?
The timeline varies depending on the amount of expansion required, individual response and whether additional orthodontic treatment is needed. Your provider can give you a more personalised estimate after assessment.
Can MARPE straighten my teeth?
MARPE is primarily intended to widen the upper jaw, not to straighten teeth on its own. Braces or clear aligners may be needed to address tooth alignment and refine the bite after expansion.
Is MARPE an alternative to jaw surgery?
For selected patients, MARPE may be considered as an alternative to surgically assisted expansion. However, it does not work in every case, and some patients may still require surgery to achieve the planned result.
Is MARPE suitable for everyone with a narrow upper jaw?
No. Suitability depends on your skeletal anatomy, dental condition, treatment goals and other clinical factors. An individual assessment is needed to determine the most appropriate approach.
